Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: Observed an expired permit in the facility. Upon further investigation, EHS observed that the facility had been inspected this year. CA - Legal notice issued.
In-use utensils: properly stored — Inspector Note: Observed knives stored between the fridge and the prep table. PIC moved the knives to the sink.
Physical facilities installed, maintained, and clean — Inspector Note: Observed heavy grease build up on the ceiling tiles in the ktichen. CA - PIC will either clean or replace the tiles in the kitchen.
Insects, rodents, and animals not present — Inspector Note: Observed the screen door at the back of the kitchen was open. Upon further investigation, EHS observed that the door’s screen was damaged and when fully closed that is a gap between the door and the frame. CA - PIC will repair or replace the screen door so that it is fully flush to the frame.
Proper cold holding temperatures — Inspector Note: Observed several TCS food items above 41 F. See temp log for readings. PIC will discarded any food prepared before today.
Physical facilities installed, maintained, and clean — Inspector Note: Observed dirty A/C filter grate on ceiling inside the kitchen that needs cleaning
Food properly labeled; original container — Inspector Note: Observed multiple food items stored unlabeled. PIC labeled all food with common name.
Contamination prevented during food preparation, storage, display — Inspector Note: Observed containers of food stored on the floor throughout kitchen. Must be stored at least 6 inches off the floor.
Wiping cloths: properly used and stored — Inspector Note: observed several wiping cloths stored on equipment throughout main kitchen area. When not in use, wiping cloths must be stored in a sanitizing solution.
In-use utensils: properly stored — Inspector Note: Observed in-use utensil in non TCS food not stored with handle above top of food within a closed container. CA- During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored in the food with their handles above the top of the food and the container.
Physical facilities installed, maintained, and clean — Inspector Note: Observed build up on ceiling tiles throughout kitchen. Advised PIC to clean ceiling tiles regularly.
Response procedures for vomiting & diarrheal events — Inspector Note: No copy of clean up procedure for vomit or diarrheal events on file. Advised PIC of the Clean up procedure.
Food separated and protected — Inspector Note: Observed raw animal foods not properly separated from each other in walk in cooler unit during preparation. CA- Food shall be protected from cross contamination by separating raw animal foods during preparation, holding, and display using the proper storage of chicken at the bottom, then beef, pork, seafood and at the top ready to eat food.
Food stored covered — Inspector Note: Observed multiple items throughout kitchen stored uncovered. PIC covered all food items and advised to ensure all food is stored covered.
Proper cold holding temperatures — Inspector Note: Observed leafy greens on prep line at 45F. CA-PIC discarded TCS foods above 41F and advised to ensure TCS foods are stored below 41F.
