CA: Operator tasked team member to remove all boxes from floor during inspection. 511-6-1.05(10)(e)1&3 - Single-Service/Single-Use items stored 6" off floor in clean, dry location (C) (e) Equipment, Utensils, Linens, and Single-Service and Single-Use Articles. 1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor. 3. Single-service and single-use articles shall be stored as specified under paragraph 1 of this subsection and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used.
Plumbing installed; proper backflow devices — Inspector Note: 511-6-1.06(2)(r)] Observed water backing up from floor drain underneath dump sink at server station in kitchen. CA: Operator contacted maintenance personnel during inspection to clear drain line, sink placed OOO during inspection. Plumbing installed; proper backflow devices — Inspector Note: Observed no Air gap installed on prep sink next to server station in kitchen. 511-6-1.06(2)(d) - Backflow Prevention, Air Gap (P) (d) Backflow Prevention, Air Gap. An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than 1 inch (25 mm). P Must correct within 10 days.
Physical facilities installed, maintained, and clean — Inspector Note: Observed wall coving in disrepair across from server station in kitchen. Must repair within 10 days. 511-6-1.07(2)(e) - Floor Covering, Mats & Duckboards (C) (e) Floor Covering, Mats and Duckboards. Mats and duckboards shall be designed to be removable and easily cleanable.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Hood filters not equipped with a drip tray. 511-6-1.05(1)(a) - Materials, General Requirements (P, C) (1) Materials. (a) General Requirements. Utensils and food-contact surfaces of equipment shall be made of materials that do not allow the migration of deleterious substances or impart colors, odors, or tastes to food and under normal use conditions, and shall be: P safe P, durable, corrosion-resistant, and nonabsorbent. In addition, materials shall be sufficient in weight and thickness to withstand repeated warewashing and shall be finished to have a smooth, easily cleanable surface. Materials shall be resistant to pitting, chipping, crazing, scratching, scoring, distortion and decomposition. CA: Provide a drip tray for hood within 30 days.
Nonfood-contact surfaces clean — Inspector Note: Observed soiled prep top coolers door handles. 511-6-1.05(7)(d) - Nonfood-Contact Surfaces (C) (d) Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food service establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and backflow prevention is required by law, by: 1. Providing an air gap; P or 2. Installing an approved backflow prevention devices. CA: must provide a air gap on prep sinks within 10 days.

Wall and ceiling covering materials shall be nonabsorbent, light colored, and attached so that they are easily cleanable. 2. Except in areas used only for dry storage, concrete, porous blocks, or bricks used for indoor wall construction shall be finished and sealed to provide a smooth, nonabsorbent, easily cleanable surface. (g) Walls and Ceiling, Attachments. 1. Except as specified in paragraphs 2 of this subsection, attachments to walls and ceilings such as light fixtures, mechanical room ventilation system components, vent covers, wall mounted fans, decorative items, and other attachments shall be easily cleanable. 2. In a consumer area, wall and ceiling surfaces and decorative items and attachments that are provided for ambiance need not meet this requirement if they are kept clean. CA: Must replace missing and damaged ceiling tiles within 30 days.
