Advised PIC to have pest control come again to eliminate pests. 511-6-1.07(5)(k) - Controlling Pests (Pf, C) (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and 4. Eliminating harborage conditions.
Response procedures for vomiting & diarrheal events — Inspector Note: EHS was not presented with fecal/vomit kit or a signage for vomiting/ diarrheal response procedures during inspection. Advised PIC to post signage for response procedures or have a laminated document outlining the step-by-step process for safely isolating the area, removing the waste, and using an approved sanitizer. 511-6-1.03(6) - Clean-up of Vomiting and Diarrheal Events(Pf) (6) Responding To Contamination Events. A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.Pf
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed mold-like substance on the lip of the ice machine in the main kitchen. Toxic substances properly identified, stored, used — Inspector Note: Observed 6 hour stem wick fuel being stored next to sauces and dry food products in dry storage area. Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, and single-service and single-use articles by: 1. Separating the poisonous or toxic materials by spacing or partitioning; P and 2. Locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, and single-service or single-use articles. This requirement does not apply to equipment and utensil cleaners and sanitizers that are stored in warewashing areas for availability and convenience if the materials are stored to prevent contamination of food, equipment, utensils, linens, and single-service and single-use articles. Contamination prevented during food preparation, storage, display — Inspector Note: Observed food stored on the floor of the emergency food storage room. CA - PIC will ensure that food stored off of the ground.
Plumbing installed; proper backflow devices — Inspector Note: Observed expired water filters for kitchen and memory care. CA - PIC will ensure that these filters are replaced.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed a black mold like substance on the baffle and walls of the ice machine again. PIC had an employee discard the ice then clean and sanitize the machine.
Proper cold holding temperatures — Inspector Note: Observed frozen potentially hazardous food slacking at greater than 41 degrees Fahrenheit. PIC discarded the food.
