CA: Operator removed all boxes from floor during inspection. 511-6-1.05(10)(e)1&3 - Single-Service/Single-Use items stored 6" off floor in clean, dry location (C) (e) Equipment, Utensils, Linens, and Single-Service and Single-Use Articles. 1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor. 3. Single-service and single-use articles shall be stored as specified under paragraph 1 of this subsection and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used.
Nonfood-contact surfaces clean — Inspector Note: Observed build up of mold like substance on shelving within lowboy reach in cooler in food preparation area in kitchen. Observed soiled reach in cooler gaskets throughout cook line in kitchen. CA: Operator instructed employee to properly clean/sanitize all non food contact surfaces during inspection. 511-6-1.05(7)(d) - Nonfood-Contact Surfaces (C) (d) Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
Garbage/refuse properly disposed; facilities maintained — Inspector Note: Observed garbage build up within the dumpster enclosure and dumpsters overflowing.Refuse shall be discarded frequently enough to prevent build up. CA: Operator must Clean dumpster area frequently and have all discarded items removed. 511-6-1.06(5)(r),(s) - Frequency, removal of refuse, recyclables and returnables often enough to minimize odors and attracting pests; Receptacles or Vehicles, removal from premises in approved manner (C) (r) Frequency. Refuse, recyclables, and returnables shall be removed from the premises at a frequency that will minimize the development of objectionable odors and other conditions that attract or harbor insects and rodents. (s) Receptacles or Vehicles. Refuse, recyclables, and returnables shall be removed from the premises by way of: 1. Portable receptacles that are constructed and maintained according to law; or 2. A transport vehicle that is constructed, maintained, and operated according to law.

CA: Advised manager, Michael Jones to post the top of the current inspection report in the drive-thur window (at the POS window) 511-6-1.02(1)(d) - Displaying of the Inspection Report (C) 2. Food service establishments with drive-thru windows will post the current inspection report, and also have the inspection report posted so that a minimum of the top one-third of a copy of the current inspection report is visible through each window allowing customers to easily read the score, date of inspection and establishment information.
Single-use/single-service articles: properly stored, used — Inspector Note: Single use cups at the tea areas, are not being kept in the original protective package or are not being stored by using other means that afford protection from contamination until they are used. Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed reach in drawer cooler not holding food at the proper temperatures. COS: All food (shelled eggs and three cartons of liquid eggs) was discarded and Found that the unit (drawer reach in cooler) had been un-plugged during the morning cleaning. Rechecked the temperature of the drawer reach in cooler at the end of my inspection. Unit ambient holding temperature was at 37 degrees F. 511-6-1.05(6)(a) - Good Repair & Proper Adjustment (C) (6) Maintenance and Operation. (a) Good Repair and Proper Adjustment.

Plumbing installed; proper backflow devices — Inspector Note: Observed water leaking at plumbing fixture of the 4 compartment sink. .COS: Manager had leak repaired during my inspection. 511-6-1.06(2)(r) - System Maintained in Good Repair (P, C) (r) System Maintained in Good Repair. A plumbing system shall be repaired according to law; P and maintained in good repair.
