Insects, rodents, and animals not present — Inspector Note: During the inspection EHS observed the establishment had a heavy presence of gnats throughout the establishment. CA: PIC was advised to have pest control to come out and service the establishment.
Adequate handwashing facilities supplied & accessible — Inspector Note: Observed the hand sink in the back prep area as not functional. EHS attempted to wash his hands and no water came out when knobs were turned. CA: PIC was advised to have the sinks repaired so that they are operable and able to wash their hands in that prep area. 511-6-1.06(2)(o) - Using a Handwashing Sink- Operation & maintenance (Pf) (o) Using a Handwashing Sink. 1. Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: During routine inspection EHS observed the establishment failed to post their most recent inspection report. instead they had their previous report posted. CA: PIC was advised to have the most current posted at all times. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.

Certified Food Protection Manager — Inspector Note: Observed failure to provide an updated certified food safety manager certificate for public view. CA: At least one employee that has supervisory and management responsibility and the authority to direct and control food preparation and service shall be a certified food safety manager who has shown proficiency of required information through passing a test that is part of an accredited program that conforms to the national standards for organizations that certify individuals. Certified Food Safety Managers must be designated to one food service establishment only and maintain and renew certification in accordance with the requirements of the examination taken.
Proper cold holding temperatures — Inspector Note: Observed the establishment failed to maintain the proper cold holding temp of 41 degrees or below in their walk in cooler. CA: PIC was advised to have their cooler to be set at colder temp or use shallow pans to aid in keeping the foods cold at the required temp or to slightly cover the foods to allow for air to circulate through the food for proper cooling. Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above.
