The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed a buildup of grease on the vent hood. CA: Exhaust Ventilation Hood Systems. Exhaust ventilation hood systems in food preparation and warewashing areas, including components such as hoods, fans, guards, and ducting shall be designed to prevent grease or condensation from draining or dripping onto food, equipment, utensils, linens, and single-service and single-use articles.
Management knowledge, responsibilities, reporting — Inspector Note: Failed to provide proof of employee health agreement policy for each employee. CA: make sure food employees and conditional employees are informed in a verifiable manner of their responsibility to report, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food. COS: Department provided education and signature pages for verification.
Food-contact surfaces: cleaned & sanitized — Inspector Note: During routine EHS observed the ice machine deflector plate soiled with old black organic matter. CA: Food contact surfaces shall be clean to sight and touch.

CA: (b) Working Containers, Common Name. The most current inspection report shall be prominently displayed in public view at all times, within fifteen feet of the front or primary public door and between five feet and seven feet from the floor and in an area where it can be read at a distance of one foot away.
Single-use/single-service articles: properly stored, used — Inspector Note: During routine inspection EHS observed several ramekins resting in food (seasonings and dressings) found in containers and prep top cooler. COS: PIC discarded the ramekins found in food and was informed that Single-service and single-use articles may not be reused.
Insects, rodents, and animals not present — Inspector Note: Observed gnats flying in the main kitchen in dry storage area. CA: Advised PIC to routinely get pest control inspections to prevent pests.

CA: Advised PIC to add an asterisk, disclosure and consumer advisory to all the food items on the menu that can be served raw or undercooked. Asterisks next to the menu items with animal-derived foods food items that can be served raw or undercooked(burger, eggs etc.); 2. A disclosure for ALL RAW OR UNDERCOOKED food items that informs consumers of the significantly increased risk of consuming such foods (A description of the animal-derived foods, such as “eggs coked any style,” and “hamburgers (can be cooked to order)); 3. Asterisk next to a consumer advisory at the footnote of the page ( ex. “ *Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness* ” ; OR “ *Consuming raw or undercooked meats, poultry, seafood, shellfish, or eggs may increase your risk of foodborne illness, especially if you have certain medical conditions* ”).
