Physical facilities installed, maintained, and clean — Inspector Note: Observed cracked and missing tiles in the kitchen floor. CA advised PIC that the floors need to be repaired and that all physical facilities should be maintained in good condition.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed black residue on the interior surface of the ice machine. Advised PIC to have the ice machine sanitized and cleaned.
