Observed containers of boiled eggs and sausage with direct contact of ice inside the container. 511-6-1.04(4)(i) - Storage or Display of Food in Contact with Water or Ice (C) (i) Storage or Display of Food in Contact with Water or Ice. 2. Except as specified in paragraphs 3 and 4 of this subsection, unpackaged food may not be stored in direct contact with undrained ice. PIC advised to separate food and ice.
Proper date marking and disposition — Inspector Note: Observed failure to discard multiple food items past date marking date (container of sausage, snow crab, lobster) in the walk and reach in cooler. 511-6-1.04(6)(h) - Ready-To-Eat Time/Temperature Control for Safety Food, Disposition (P) (h) Ready-to-Eat, Time/Temperature Control for Safety Food, Disposition. 1. A food that requires datemarking shall be discarded if it: (i) Exceeds 7 days, not including the time that the product is frozen; P (ii) Is in a container or package that does not bear a date or day; P or (iii) Is inappropriately marked with a date or day that exceeds 7 days . P PIC discarded food items and EHS explained how to properly date mark food items.
Proper cooling methods used: adequate equipment for temperature control — Inspector Note: Observed failure to properly cool TCS food using an approximate cooling method. Observed container of cooked chicken wings sitting on the counter at 80F. (e) Cooling Methods. 1. Cooling shall be accomplished in accordance with the time and temperature criteria specified under DPH Rule 511-6-1-.04(6)(d) by using one or more of the following methods depending on the type of food being cooled: (i) Placing the food in shallow pans; Pf (ii) Separating the food into smaller or thinner portions; Pf (iii) Using rapid cooling equipment; Pf (iv) Stirring the food in a container placed in an ice water bath; Pf (v) Using containers that facilitate heat transfer; Pf (vi) Adding ice as an ingredient; Pf or (vii) Other effective methods. Pf PIC moved food into walk in cooler.
Proper cold holding temperatures — Inspector Note: Observed failure to maintain TCS food items at 41F or below, observed container of shrimp at 55F in prep top cooler. (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. P PIC discarded food items.

(a) Good Repair. All physical facilities shall be maintained in good repair. PIC will fix the problem within 3 days.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed black mold like substance on bar soda gun. 511-6-1.05(7)(a)1 - Equipment, Food-Contact Surfaces,& Utensils (Pf) (7) Cleaning of Equipment and Utensils. (a) Equipment, Food-Contact Surfaces, and Utensils. 1. Equipment food-contact surfaces and utensils shall be clean to sight and touch. PIC advised to wash and sanitized every end of the shift.
