Item 10D Good Retail Practice

Food properly labeled; original container — Inspector Note: Observed failure to label several food items in the prep top cooler and walk in cooler. All food items that are not in their original containers and not easily identified shall be properly identified. PIC shall have all food removed from their original containers labeled.

Item 12A Good Retail Practice

Contamination prevented during food preparation, storage, display — Inspector Note: Observed food and cardboard boxes being stored directly on the floor in the walk in freezer, dry storage and walk in cooler. Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food: (i) In a clean, dry location; (ii) Where it is not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 cm) above the floor. PIC shall have all food items stored 6 inches above the floor.

Item 2-2A Critical / Risk Factor

Management knowledge, responsibilities, reporting — Inspector Note: Observed no employee health agreement form signed by employees. PIC was unaware of an employee health agreement policy. The permit holder shall require food employees and conditional employees to report to the CFSM and person in charge, information about their health and activities as they relate to diseases that are transmissible through food. A food employee or conditional employee shall report the information in a manner that allows the CFSM and person in charge to reduce the risk of foodborne disease transmission, including providing necessary additional information, such as the date of onset of symptoms and an illness, or of a diagnosis without symptoms. PIC shall have employees and self sign an employee health agreement policy.

Item 2-2B Critical / Risk Factor

Proper eating, tasting, drinking, or tobacco use — Inspector Note: Observed employee drinking juice that was not in a single service beverage cup in the main kitchen. (CA) Advised the PIC that employees are to eat and drink only in designated areas. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. However, drinking from a single service beverage cup with a secure lid and straw that is handled to prevent contamination of the employee’s hands, the container, exposed food, clean equipment, utensils and linens, unwrapped single-service and single-use articles will be allowed.

No discharge from eyes, nose, and mouth — Inspector Note: Observed employee sneezing multiple times while performing kitchen duties. CA advised PIC that Food employees experiencing persistent sneezing, coughing, or runny nose that cause discharges from the eyes, nose, or mouth may not work with exposed food; clean equipment; utensils, and linens; or unwrapped single-service articles.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed cutting boards with heavy staining and scoring. (CA) Advised the PIC to replace the cutting boards. Equipment food-contact surfaces and utensils shall be clean to sight and touch. Pf.

Proper date marking and disposition — Inspector Note: Observed failure to date mark food items prepped from previous days. Food items prepped onsite for storage shall be properly date marked. Advised PIC to discard it.
