No food safety management course has been registered for at the time of inspection. Corrective action - PIC advised to have at least one employee with supervisory responsibility to register for and complete an approved food safety management course. Food service establishments shall have in its employ a Certified Food Safety Manager (CFSM) as specified in paragraph (b) of this subsection to ensure food safety is being managed within the food service establishment during all hours of operation as specified within paragraph (d) of this subsection.
Food-contact surfaces: cleaned & sanitized — Inspector Note: observed discolored and heavily scored cutting board in the main kitchen attached to the prep-top cooler. CA: PIC was instructed to replace the cutting board. Equipment food-contact surfaces and utensils shall be clean to sight and touch
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed quat chemical sanitizer of the 3 compartment sink under the concentration limit during the time of the inspection. Quat sanitizer must be between 200-400ppm. COS. PIC replaced chemical sanitizer. Currently at 400ppm. Advised PIC to limit usage of too much chemical sanitizer must be between 200-400ppm. Corrected On-Site.
Proper cold holding temperatures — Inspector Note: Observed facility failed to properly hold time/temperature control for safety foods (Milks 55 degrees F) at 41 degrees or below in the front food service RIC. TCS foods shall be held 41 degrees F or below. PIC informed EHS that the TCS foods were in the RIC cooler for over 24hrs. CA: PIC discarded the food and was advised to have the RIC serviced so that it can maintain cold holding temps.
Wiping cloths: properly used and stored — Inspector Note: Observed wiping cloths not stored in sanitizer between uses (several stored dry in bowl in front food service area, one stored dry on countertop in front food service area, one stored dry on shelving in main kitchen area). Corrective action - PIC advised to store wiping cloths between uses in sanitizing solution.

Management knowledge, responsibilities, reporting — Inspector Note: The PIC could not provide documentation or otherwise satisfactorily demonstrate during the inspection, that all food employees and conditional employees are informed of their responsibility to report to management information about their health and activities as it relates to diseases that are transmissible through food.
Response procedures for vomiting & diarrheal events — Inspector Note: Establishment does not have established procedures for employees to follow when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food establishment. COS- EHS provided establishment with emailed clean up procedures using bleach. PIC will create and obtain vomit and fecal matter clean up kit, also create procedures for clean up and disposition. 511-6-1.03(6) - Clean-up of Vomiting and Diarrheal Events(Pf) (6) Responding To Contamination Events. A food establishment shall have written procedures for employees to follow and an EPA registered disinfectant effective against Norovirus available on-site at the food service establishment when responding to vomiting or diarrheal events that involve the discharge of vomitus or fecal matter onto surfaces in the food service establishment. The procedures shall address the specific actions employees must take to minimize the spread of contamination and the exposure of employees, consumers, food, and surfaces to vomitus or fecal matter.Pf
