Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed scored, stained cutting boards in use at the preptop cooler and stored in the rear of the facility near the warewashing sink. Corrective action - PIC reported that new cutting boards were being purchased for the establishment. 511-6-1.05(2)(c) - Multiuse Food-Contact Surfaces (Pf) (c) Multiuse Food-Contact Surfaces. 1. Physical facilities installed, maintained, and clean — Inspector Note: Observed debris and food residue on floor of walk-in freezer. Observed dusty ceiling vents and ceiling tiles in food prep area of main kitchen. Corrective action- PIC advised to have floors, walls, and ceilings cleaned often enough to keep them clean. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products.

Food properly labeled; original container — Inspector Note: observed several squeeze bottles and food containers lacking labeling with common name of items stored in them. CA: (d) Food Storage Containers, Identified with Common Name of Food. Except for containers holding food that can be readily and unmistakably recognized, such as dry pasta, working containers holding food or food ingredients that are removed from their original packages for use in the food establishment, such as cooking oils, flour, herbs, potato flakes, salt, spices, and sugar shall be clearly and legibly identified, in English, with the common name of the food.

Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed gaskets in the RIC of the main kitchen area that houses the eggs broken. CA: Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2)of this Rule. 2. Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.

Food in good condition, safe, and unadulterated — Inspector Note: observed red onions and lettuce stored in the WIC that was spoiled and molded. COS: PIC discarded the food presented to they by EHS.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed the ice scoop holder with black organic matter resting at the bottom of the container. There were two scoops in the bin (one small, one large) the smaller scoop was also had black organic matter on it. CA: PIC was advised to have all items cleaned and sanitized.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed the sanitizer in the back kitchen and the sanitation bucket over the 400ppm quat range. The solution that poured from the wall did not have enough presence of water to dilute the contents. There was so much sanitizer present that it caused coughing. CA: Advised PIC to manually add water to the solution and test the chemical solutions after to insure that it is within regulation.
