In-use utensils: properly stored — Inspector Note: Observed a plastic bowl stored inside the container of chicken flour in the main kitchen area. Observed stirring spoons stored inside a container of room temperature water in the front coffee service area. Corrective action - Food employees advised to store dispensing utensil for flour outside of the container or in such a way that the handle doesn’t touch the food. Food employees advised to store stirring spoons for coffee drinks on a clean surface or in water with a temperature at or above 135*F. 511-6-1.04(4)(k) - In-Use Utensils, Between-Use Storage (C) (k)Storage of In-Use Utensils. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: 1.Except as specified under paragraph 2 of this subsection, in the food with their handles above the top of the food and the container; 2.In food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon; 3.On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at a frequency specified under DPH Rule 511-6-1-.05(7)(b) and (8)(a); 4.In running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes; 5. In a clean, protected location if the utensils, such as ice scoops, are used only with a food that is not time/temperature control for safety food; or 6. In a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at a frequency specified under DPH Rule 511-6-1-.05(7)(b)3(vi).
Utensils, equipment and linens: properly stored, dried, handled — Inspector Note: Observed plates stored as clean on shelving near dishwasher with the food-contact surface up. Corrective action - Food employees advised to invert plates during storage or store plates in such a way as to protect the food-contact surface. 511-6-1.05(10)(e)1,2,4 - Equipment, Utensil, Linens, stored 6" off floor in clean, dry location (C) (e) Equipment, Utensils, Linens, and Single-Service and Single-Use Articles. 1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor. 2. Clean equipment and utensils shall be stored as specified under paragraph 1 of this subsection and shall be stored: (i) In a self-draining position that allows air drying; and (ii) Covered or inverted. 4. Items that are kept in closed packages may be stored less than 6 inches (15 centimeters) above the floor on dollies, pallets, racks, and skids that are designed as specified under subsection (2)(hh) of this Rule.
PIC present, demonstrates knowledge, performs duties — Inspector Note: Observed no PIC present at time of inspection. Corrective action - PIC shall be present to answer food safety questions and ensure employees are following sound food safety practices in the establishment. Corrected on site - PIC showed up at the end of the inspection. 511-6-1.03(2) - PIC present (Pf) There must be a person in charge on the premises of the food service establishment at all times. Adequate handwashing facilities supplied & accessible — Inspector Note: Observed a steel wool sponge stored in the hand washing sink near the dishwasher in the main kitchen area. Corrected on site - sponge removed. A handwashing facility may not be used for purposes other than handwashing.
Food stored covered — Inspector Note: Observed numerous containers of food items stored uncovered in reach-in cooler (package of turkey bacon, containers of seasoned raw chicken, containers of pancake batter) and on preptop cooler (containers of sliced strawberries, container of blueberries, container of whipped cream). Corrected on site - food employee provided lids and wrap to cover food containers during storage. Observed ice stored uncovered in bin in coffee bar area. Corrective action - Food employees advised to provide cover for ice bin when ice not actively being dispensed. 511-6-1.04(4)(c)1(iv) - Packaged & Unpackaged Food, food stored covered(C) (iv) Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination;
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed dark spots with the appearance of mold on the interior of the ice machine in the main kitchen area. Corrective action - Food employees advised to perform detailed cleaning on facility ice machine. Observed debris on plates stored as clean on shelving near the dishwasher in the main kitchen area. Corrective action - Food employees advised to invert plates or otherwise store clean plates to prevent contamination of the food-contact surface. 511-6-1.05(7)(a)1 - Equipment, Food-Contact Surfaces,& Utensils (Pf) (7) Cleaning of Equipment and Utensils. Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed containers that had cracks and not in good repair. CA- all containers shall be free of breaks and cracks. 511-6-1.05(2)(h) - Nonfood-Contact Surfaces (C) (h) Nonfood-contact Surfaces. Nonfood-contact surfaces shall be free of unnecessary ledges, projections, and crevices, and designed and constructed to allow easy cleaning and to facilitate maintenance.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed multiple cutting boards stained and with gashes in the main kitchen. CA- cutting boards shall be able to be cleaned. 511-6-1.05(6)(b) - Cutting Surfaces (C) (b) Cutting Surfaces. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they cannot be resurfaced.
Warewashing facilities: installed, maintained, used; test strips — Inspector Note: Observed no chlorine test strips for the dish machine. Nonfood-contact surfaces clean — Inspector Note: Observed debris on equipment throughout the main kitchen, shelves, inside the coolers etc. CA- all nonfood contact surfaces of equipment shall be cleaned. 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. 3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Hands clean and properly washed — Inspector Note: Observed an employee wash hands without any soap, observed an employee wash hands with gloves still on their hands. COS- employees rewashed hands following the correct cleaning procedure. 511-6-1.03(5)(b) - Cleaning Procedure (P) (b) Cleaning Procedure. 1. Except as specified in paragraph 4 of this subsection, food employees shall clean their hands and exposed portions of their arms, including surrogate prosthetic devices for hands or arms, for at least 20 seconds, using a cleaning compound in a handwashing sink that is properly equipped. P 2. Food employees shall use the following cleaning procedure in the order stated to clean their hands and exposed portions of their arms, including surrogate prosthetic devices for hands and arms: (i) Rinse under clean, running warm water; P (ii) Apply an amount of cleaning compound recommended by the cleaning compound manufacturer; P (iii) Rub together vigorously for at least 10 to 15 seconds while: (I) Paying particular attention to removing soil from underneath the fingernails during the cleaning procedure, P and (II) Creating friction on the surfaces of the hands and arms or surrogate prosthetic devices for hands and arms, finger tips, and areas between the fingers; P (iv) Thoroughly rinse under clean, running warm water; P and (v) Immediately follow the cleaning procedure with thorough drying using disposable paper towels, a continuous towel system, or a heated-air hand drying device. P 3. To avoid recontaminating their hands or surrogate prosthetic devices, food employees may use disposable paper towels or similar clean barriers when touching surfaces such as manually operated faucet handles on a handwashing sink or the handle of a restroom door. 4. If approved and capable of removing the types of soils encountered in the food operations involved, an automatic handwashing facility may be used by food employees to clean their hands or surrogate prosthetic devices.
