PIC present, demonstrates knowledge, performs duties — Inspector Note: Upon arrival no PIC present CA: There must be a person in charge on the premises of the food service establishment at all times
Management knowledge, responsibilities, reporting — Inspector Note: Employees where unable to provided verifiable documentation of responsibilities for PIC (Employee health agreement documentation) CA: 511-6-1.03(2)(o) - Person-in-Charge Duties (Pf) Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with the Chapter, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food;
Proper date marking and disposition — Inspector Note: observed several metal containers with food items in them located in the Reach In Coolers (RIC) of the main kitchen without any datemarking. CA: refrigerated, ready-to-eat, time/temperature control for safety food prepared and held in a food establishment for more than 24 hours shall be clearly marked to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, when held at a temperature of 41°F (5°C) or below for a maximum of 7 days. The day of preparation shall be counted as Day 1.
Warewashing facilities: installed, maintained, used; test strips — Inspector Note: . Observed PIC inability to provide test strips for quat solution. CA: A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided.
Nonfood-contact surfaces clean — Inspector Note: Observed the RIC gaskets in the main kitchen with built up food accumulations. CA: nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

Food stored covered — Inspector Note: Observed in the kitchen failure to store food items covered in RIC across from the grill and fryer. COS: PIC made cooks cover chicken and cheese found uncovered with lids.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed ice machine baffle covered with black mold like substance. Equipment food-contact surfaces and utensils shall be clean to sight and touch. CA: advised PIC to discard the ice and to not use the machine until it gets serviced
Consumer advisory provided for raw and undercooked foods — Inspector Note: during the routine inspection I observed that the establishment menu did not have a consumer advisory report on it after being told by employee that some customers request their burgers to be cooked medium rare. CA: if an animal food such as beef, eggs, fish, lamb, pork, poultry, or shellfish is served or sold raw, undercooked, or without otherwise being processed to eliminate pathogens, either in ready-to-eat form or as an ingredient in another ready-to-eat food, the permit holder shall inform consumers of the significantly increased risk of consuming such foods by way of a disclosure and reminder.
