Food properly labeled; original container — Inspector Note: Multiple containers of Time/Temperature Control for Safety (TCS) foods (including various prepared stews and sauces) observed in the walk-in cooler/prep area without proper labels identifying the common name of the food or the preparation/discard date. All ready-to-eat TCS foods prepped on-site must be clearly labeled and date-marked. Advised PIC to have food items labeled and date-marked. 511-6-1.04(7)(c) - Food Labels (Pf, C) (c) Food Labels. 1. Food packaged in a food service establishment, that is not provided by a foodservice employee upon consumer request, shall be labeled as specified in law, including 21 CFR 101- Food labeling, and 9 CFR 317 Labeling, marking devices, and containers. 2. Label information shall include: (i) The common name of the food, or if there is no common name, an adequately descriptive identity statement; (ii) If made from two or more ingredients, a list of ingredients and sub-ingredients in descending order of predominance by weight, including a declaration of artificial color or flavors and chemical preservatives, if contained in the food.

Proper date marking and disposition — Inspector Note: Multiple containers of cooked lamb and goat observed in the walk-in-cooler without a date mark. Per the PIC, the items were prepared on 6/5/26. Advised PIC to have all foods kept for more than 24 hours to have a proper label and date-mark. 511-6-1.04(6)(g) - Ready-To-Eat Time/Temperature Control for Safety Food, Date Marking (Pf) (g) Ready-to-Eat Time/Temperature Control for Safety Food, Date Marking 1. Item 17C Good Retail Practice

Physical facilities installed, maintained, and clean — Inspector Note: Observed coving for wall behind grills coming off in main kitchen. In food service establishments in which cleaning methods other than water flushing are used for cleaning floors, the floor and wall junctures shall be coved and closed to no larger than one thirty-second inch (1 mm). 2. The floors in food service establishments in which water flush cleaning methods are used shall be provided with drains and be graded to drain, and the floor and wall junctures shall be coved and sealed. Coving was fixed during inspection.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed a chlorine and Quats concentration below the recommended range. The recommended ranges are 50 - 100 ppm and 150 - 400 ppm respectively. 1. Item 12A Good Retail Practice

Contamination prevented during food preparation, storage, display — Inspector Note: Observed oil and old drums of vegetables stored on the floor. 1. Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food: (i) In a clean, dry location; (ii) Where it is not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 cm) above the floor. PIC will ensure that all food is stored off of the floor.

Food stored covered — Inspector Note: Observed several containers of cooked food in fridges and seasonings that were uncovered. (iv) Except as specified under paragraph 2. of this subsection and when cooling as specified in DPH Rule 511-6-1-.04(6)(e)2.(ii), storing the food in packages, covered containers, or wrappings, except for loosely covered or uncovered containers in which food is being cooled if protected from overhead contamination; PIC will ensure that all food is covered to protect from contamination.

Proper cold holding temperatures — Inspector Note: Observed TCS food above 41 F on the grill prep line. See temp log for readings. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. P PIC discarded the tested items.
