The existing servsafe is expired on 11/2025. PIC advised to have servsafe certified employee present and post the valid servsafe in public display. 511-6-1.03(3)(c) - Certification Documentation. The original CFSM certificate shall be posted in public view in each food service establishment. An additional copy shall be retained on file at the food service establishment at all times, and shall be made available for inspection by the Health Authority.

Adequate ventilation and lighting; designated areas used — Inspector Note: Observed employee food, coats and hats in multiple places in the main kitchen and dry storage. PIC removed all the employee stuff and was advised to have employee designated areas to store their stuff. 511-6-1.07(4)(b) - Designated Areas for Employee Activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (C) (b) Designated Areas for Employee Activity. 1. Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. 2. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.

Proper cold holding temperatures — Inspector Note: Observed several cheeses in prep top cooler held at 45F ,48F and when checked, ambient temperature of the cooler was 46F. PIC discarded the cheeses and was advised to keep TCS foods below 41F or above 135F and to get the cooler fixed ASAP.

Time as a public health control: procedures and records — Inspector Note: Observed whipped butter held at 70F and when asked PIC said that the butter is held for 4 hours. Butter did not have a label on when it is prepared and no procedures were available on holding butter based on time. PIC discarded and advised to label butter and have written procedures for time holding butter. (i) Except as specified in paragraph 2.(ii) of this subsection, the food shall have an initial temperature of 41°F (5°C) or less when removed from cold holding temperature control, or 135°F (57°C) or greater when removed from hot holding temperature control;P (ii) The food may have an initial temperature of 70°F (21°C) or less if; (I) It is a ready-to-eat fruit or vegetable that upon cutting is rendered a time/temperature control for safety food, or (II) It is a ready-to-eat hermetically sealed food that upon opening is rendered a time/temperature control for safety food, (III) The food temperature does not exceed 70°F (21°C) within a maximum time period of 4 hours from the time it was rendered a time/temperature control for safety food; and (IV) The food is marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is rendered a time/temperature control for safety food as specified in paragraph 2.(ii)(I) and (II). (iii) The food shall be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control;Pf (iv) The food shall be cooked and served, served at any temperature if ready-to-eat, or discarded, within 4 hours from the point in time when the food is removed from temperature control;P and (v) The food in unmarked containers or packages, or marked to exceed a 4-hour limit shall be discarded.P 3.
