Utensils, equipment and linens: properly stored, dried, handled — Inspector Note: Observed dishes stacked wet and not allowed to air dry. COS- dishes were unstacked to allow air drying. 511-6-1.05(10)(a) - Equipment & Utensils, Air-Drying Required (C) (10) Protection of Clean Items. (a) Equipment and Utensils, Air-Drying Required. After cleaning and sanitizing, equipment and utensils: 1. Shall be air-dried or used after adequate draining before contact with food; and 2. May not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed the cutting boards on the preptop cooler stained and with gashes at the time of the inspection. CA- cutting boards shall be able to be cleaned and if unable should be replaced or resurfaced. 511-6-1.05(6)(b) - Cutting Surfaces (C) (b) Cutting Surfaces. Surfaces such as cutting blocks and boards that are subject to scratching and scoring shall be resurfaced if they can no longer be effectively cleaned and sanitized, or discarded if they cannot be resurfaced.
Warewashing facilities: installed, maintained, used; test strips — Inspector Note: Observed a high temperature dish machine in the bar area an no irreversible temperature indicator to check the utensil surface temperature.511-6-1.05(3)(h),(i),(j) - Temperature Measuring Device, Manual Warewashing; Sanitizing Solutions, Testing Device (Pf) (h) Temperature Measuring Devices, Manual and Mechanical Warewashing. 2. Nonfood-contact surfaces clean — Inspector Note: Observed dishes stored as clean with residue on the outsides of the containers. COS- dishes were rewashed. 511-6-1.05(7)(d) - Nonfood-Contact Surfaces (C) (d) Nonfood-Contact Surfaces. Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed dishes being washed in the dish machine in the main kitchen and the sanitizer concentration reading at 0ppm. COS- dishes were rewashed manually in the 3 compartment sink. 511-6-1.05(6)(n) - Manual and Mechanical Warewashing Equipment, Chemical Sanitization-Temperature, pH, Concentration, Hardness (P,Pf) (n) Manual and Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration, and Hardness. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions P, and shall be used as follows: 1. Adequate ventilation and lighting; designated areas used — Inspector Note: Observed mechanical ventilation on the outside of the building not sufficient in capacity to prevent smoke and fumes. CA- ventilation shall be sufficient to prevent smoke from going into the apartments overhead. -6-1.07(3)(g) - Mechanical Ventilation, sufficient capacity to prevent buildup of heat, steam, smoke, fumes & odors (C) (g) Mechanical Ventilation. If necessary to keep rooms free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke, and fumes, mechanical ventilation of sufficient capacity shall be provided.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed the ventilation system vented to the outside and buildup of grease coming out the ventilation. CA- ventilation system shall not create a public nuisance. 511-6-1.07(5)(d) - Cleaning Ventilation System, Nuisance & Discharge Prohibition, cleaned in way not to cause contamination or create a public health hazard (C) (d) Cleaning Ventilation Systems, Nuisance and Discharge Prohibition. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.
Proper cold holding temperatures — Inspector Note: Obserevd multiple TCS items holding above 41 degrees in the WIC, and two preptop coolers. COS- items were discarded and WIC was repaired at the time of the inspection. The two preptop coolers were also getting repaired. 511-6-1.04(6)(f) - Time/Temperature Control for Safety; Cold Holding (P) (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above.
