Toilet facilities: properly constructed, supplied, cleaned — Inspector Note: Observed men's and women's restroom doors not tightly fitted. 511-6-1.07(2)(l) - Toilet Rooms, Enclosed (C) (l) Toilet Rooms, Enclosed. Except where a toilet room is located outside a food service establishment and does not open directly into the food service establishment, such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door.

Adequate ventilation and lighting; designated areas used — Inspector Note: Observed employee personal food items stored with food item intended for customers in reach-in cooler. PIC was advised to separate items and place them into a designated area. 511-6-1.07(4)(b) - Designated Areas for Employee Activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (C) (b) Designated Areas for Employee Activity. 1. Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. 2. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.

Insects, rodents, and animals not present — Inspector Note: Observed a mosquito above 3-compartment sink, a fly in ice machine, a live spider and fly in women's restroom. 511-6-1.07(5)(k) - Controlling Pests (Pf, C) (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and 4. Eliminating harborage conditions.

Proper eating, tasting, drinking, or tobacco use — Inspector Note: Observed employee drinks without straws or lids stored with packaged bread, next to clean utensils, and above stored single use items. Employees shall not use any form of tobacco products, or electronic devices that simulate tobacco smoking, while engaged in food preparation or service, nor while in areas used for equipment or utensil washing and storage, food preparation or food storage. Employees shall only use tobacco products or electronic devices that simulate tobacco smoking in approved designated areas. 2. Employees shall consume food only in approved designated areas separate from food preparation and serving areas, equipment or utensil areas and food storage areas. Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed fly on white deflector of ice machine and food debris on cleaned pans stored under grill. Also observed with excessive dish machine racks with brown accumulation. PIC removed fly and placed pans to be properly cleaned and sanitized.
