Physical facilities installed, maintained, and clean — Inspector Note: Observed the floor tile in need of repair in several areas in the kitchen.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed the unisex restroom vent and the vent over the vegetable prep sink in need of cleaning.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed black build up on the side of the interior surface of the ice machine. Advised PIC to clean the black build up from the side of the machine (COS).
Proper date marking and disposition — Inspector Note: Observed raw chicken breast stored in the lowboy cooler with an expiration date of 4/6. Advised PIC to discard the chicken (COS).
