Contamination prevented during food preparation, storage, display — Inspector Note: Observed apples not covered or protected displayed in the food service area. Also observed a box of sugar packets stored on the floor. All food and food contact surfaces must be stored 6" above the floor> 511-6-1.04(4)(u) - Food Display (P) (u) Food Display. 1. Except for nuts in the shell and whole, raw fruits and vegetables that are intended for hulling, peeling, or washing by the consumer before consumption, food on display shall be protected from contamination by the use of packaging; counter, service line, or salad bar food guards, display cases, or other effective means. P 2. Protective devices for counters, serving lines, salad bars and other similar food displays in food service establishments shall be designed and constructed so as to intercept contaminants which may be expelled from the customer's mouth or nose. 3. All food, whether on display, being prepared for service or placed for consumer self-service must be protected from contamination from consumers standing or sitting within eight feet of the food, except that table side finishing as approved by the Health Authority and hibachi grills will be allowed when food preparation is for immediate service.

Personal cleanliness — Inspector Note: Observed hair restraints not provided for staff during food preparation. 511-6-1.03(5)(j) - Hair Restraints (C) (i) Hair Restraints. 1. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. 2. This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles.

Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed rusted shelf in the reach in cooler. The shelf must be replaced. A splash guard must be installed between the preparation sink and handsink. Signage must be installed to differentiate between the two sinks. 511-6-1.05(1)(i) - Nonfood-contact Surfaces (C) (i) Nonfood-contact Surfaces. Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.

Warewashing facilities: installed, maintained, used; test strips — Inspector Note: Observed chlorine test strips needed to test sanitizer in the three compartment sink. Test strips provided was for a swimming pool. Stoppers not provided for the three compartment sink. Three compartment sink signage must be rearranged due to the drain board being installed on the left side of the sink.
