2 recorded inspectionsAdequate ventilation and lighting; designated areas used — Inspector Note: Observed exhaust vent cover missing over the dish machine. Excessive amount of dust was observed inside of the vent. Manager was also advised on cleaning the ceiling grids above the dish machine.
Proper cold holding temperatures — Inspector Note: Observed 6 containers of grill chicken wraps exceeding 41F. Manager discarded all 6 containers and was advised on proper cooling procedures.
Contamination prevented during food preparation, storage, display — Inspector Note: Observed clear condiment lids stored with clean utensils (spoon and tongs) and a ink pen in the food preparation area. PIC rewashed the utensils and discarded the condiment lids. Also observed a used dust pan with debris stored next to buckets of pickles. There was also a sifter stored on top of pickle buckets. 511-6-1.04(4)(z) - Miscellaneous Sources of Contamination (C) (z) Miscellaneous Sources of Contamination. Food shall be protected from contamination that may result from a factor or source not specified under paragraphs (a) through (y) of this subsection.
Personal cleanliness — Inspector Note: Observed a staff member in the kitchen area without a proper hair restraint. The employee was observed rubbing hair and using her phone in the back of house. Also observed employees preparing drinks in the front of house without proper hair restraints. Employees observed with long/loose ponytails 511-6-1.03(5)(j) - Hair Restraints (C) (i) Hair Restraints. 1. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. 2. This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles.
In-use utensils: properly stored — Inspector Note: Observed a pair of tongs stored in a container that was not inverted. PIC corrected on site by removing and rewashing the tongs.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed an employee thermos stored next to the condiments near the handsink in the front of house. PIC corrected on site by relocating the thermos. 511-6-1.07(4)(b) - Designated Areas for Employee Activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (C) (b) Designated Areas for Employee Activity. 1. Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. 2. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.
Insects, rodents, and animals not present — Inspector Note: Observed the presence of several dead flies in the bottom of the vector control light. According to the PIC, the glue boards are being replaced but the pest control technician is not cleaning or removing dead insects. CA- PIC will remove the dead insects, properly clean the vector control housing area, and notify the pest control company of our findings. PIC will email a picture of the vector control light to [REDACTED EMAIL] by 1/28/26 or once the vector control light is properly cleaned. 511-6-1.07(5)(l) - Removing Dead or Trapped Birds, Insects, Rodents, and Other Pest (C) (l) Removing Dead or Trapped Birds, Insects, Rodents, and Other Pests. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests.
