Contamination prevented during food preparation, storage, display — Inspector Note: Oberved butter stored next to the handsink. Employee observed washing hands and splashing water into the butter. PIC corrected onsite by discarding the butter. PIC will relocate the butter and toaster to prevent contamination. 511-6-1.04(4)(t) - Food Preparation (C) (t) Food Preparation. During preparation, unpackaged food shall be protected from environmental sources of contamination.

Personal cleanliness — Inspector Note: Observed a staff member waring jewelry in the front of house service area. PIC corrected on site by having the employee to remove his watch and ring. 511-6-1.03(5)(g) - Jewelry (C) (g) Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food.

Adequate ventilation and lighting; designated areas used — Inspector Note: Observed build up in the vent located in the dishwasher area. PIC will have the vent cleaned immediately to prevent further dust accumulation. 511-6-1.07(5)(d) - Cleaning Ventilation System, Nuisance & Discharge Prohibition, cleaned in way not to cause contamination or create a public health hazard (C) (d) Cleaning Ventilation Systems, Nuisance and Discharge Prohibition. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.

Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed quat sanitizer did not meet the minimum required concentration of 200 ppm to 400 ppm in the three compartment sink. PIC corrected on site by remaking the sanitizer water. 511-6-1.05(6)(n) - Manual and Mechanical Warewashing Equipment, Chemical Sanitization-Temperature, pH, Concentration, Hardness (P,Pf) (n) Manual and Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration, and Hardness. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions P, and shall be used as follows: 1. If a chemical sanitizer other than chlorine, iodine, or a quaternary ammonium compound is used, it shall be applied in accordance with the EPA-registered label use instructions ;P and 6. If a chemical sanitizer is generated by a device located on-site at the food service establishment, it shall be used as specified in 1-5 of this subsection and shall be produced by a device that: (i) complies with law as specified in sections 2(q)(1) and 12 of the Federal Insecticide, Fungicide, and Rodenticide Act (FIFRA), P (ii) complies with 40 CFR 152.500 Requirement for Devices and 40 CFR 156.10 Labeling Requirements, P (iii) displays the EPA device manufacturing facility registration number on the device, Pf and (iv) is operated and maintained in accordance with manufacturer’s instructions. Pf. 7. On-site chemical sanitizer generating equipment with active ingredients, such as copper, must be registered as pesticides. Wiping cloths: properly used and stored — Inspector Note: Observed wiping cloth buckets containing quaternary ammonium sanitizing solution did not meet the proper minimum strength requirements of 200-400 PPM. PIC remade the sanitizer buckets. Once the correction was completed by the PIC, I observed the Quaternary Ammonium sanitizer at 200 ppm

Nonfood-contact surfaces clean — Inspector Note: Observed excessive build up on shelves used to store plates above the cooked chicken service area. PIC advised to clean the area prior to placing single service articles above the cooked chicken storage area. 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. 3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Proper reheating procedures for hot holding — Inspector Note: Observed improper procedures being used for reheating for hot holding. Observed beans, macaroni and cheese, and gravy improperly reheated in the CVAP machine due to the Chubb warmer not working. All TCS product was observed at 120 degrees f. As a corrective action, PIC reheated all TCS foods observed in the hotel pan thawing to 165 degrees f. PIC will email a copy of the service work order for the repair of the Chubb warmer to [REDACTED EMAIL]. 511-6-1.04(5)(h) - Reheating for Hot holding (P) (h) Reheating for Hot Holding. 1. Except as specified under paragraphs 2, 3, and 5 of this subsection, time/temperature control for safety food that is cooked, cooled, and reheated for hot holding shall be reheated so that all parts of the food reach a temperature of at least 165°F (74°C) for 15 seconds. P 2. Except as specified under paragraph 3 of this subsection, time/temperature control for safety food reheated in a microwave oven for hot holding shall be reheated so that all parts of the food reach a temperature of at least 165°F (74°C) and the food is rotated or stirred, covered, and then allowed to stand covered for 2 minutes after reheating. P 3. Ready-to-eat time/temperature control for safety food that has been commercially processed and packaged in a food processing plant that is inspected by the food regulatory authority that has jurisdiction over the plant, shall be heated to a temperature of at least 135°F (57°C) for hot holding. P 4. Reheating for hot holding shall be done rapidly and the time the food is between the temperatures of 41°F (5°C) and 165°F (74°C) and 41°F (5°C) and 135°F (57°C) for commercially processed food, may not exceed 2 hours. P 5. Remaining unsliced portions of meat roasts that are cooked as specified under subsection (5)(a)2 of this Rule may be reheated for hot holding using the same oven parameters and minimum time and temperature conditions under which it was cooked.
