Advised PIC to make sure all areas are cleaned regularly to keep them dry. 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. 3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
Plumbing installed; proper backflow devices — Inspector Note: Observed a leaking pipe surrounded by steel wool leaking onto containers of single use items wrapped in plastic. PIC instructed to remove all wet boxes and single use items that were sealed. PIC also instructed to discard any single use articles that were potentially contaminated.
Garbage/refuse properly disposed; facilities maintained — Inspector Note: Observed the dumpster plug missing on the rear dumpster and the lid was broken on the same dumpster. 511-6-1.06(5)(o),(p) - Using Drain Plugs; Maintaining Refuse Areas (C) (o) Using Drain Plugs. Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place. (p) Maintaining Refuse Areas and Enclosures. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. 511-6-1.06(5)(n) - Covering Receptacles (C) (n) Covering Receptacles. Receptacles and waste handling units for refuse, recyclables, and returnables shall be kept covered: 1. Inside the food service establishment if the receptacles and units: (i) Contain food residue and are not in continuous use; or (ii) After they are filled; and 2. With tight-fitting lids or doors if kept outside the food service establishment.
Physical facilities installed, maintained, and clean — Inspector Note: Observed floors beneath storage shelves and the lobby in need of cleaning. Several ceiling tiles in the dining/play area contained severe water leak stains. All stained ceiling tile must be replaced after the leaks are repaired in the dining area by 8/4/26. Ceiling tile must also be replaced once the leak is repaired in the storage room. A hole was observed in the ceiling in the drive thru area. PIC will have the hole repaired. 511-6-1.07(5)(a),(b) - Good Repair, physical facilities maintained; Cleaning, Frequency & Restrictions, cleaned often enough to keep them clean (C) (5) Maintenance and Operation. (a) Good Repair. All physical facilities shall be maintained in good repair. (b) Cleaning, Frequency and Restrictions. 1. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. 2. Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing. 3. If present, playground equipment and associated areas shall be maintained in a clean and sanitary condition. In addition, a cleaning and sanitizing plan along with cleaning and sanitizing records for playground equipment and associated areas shall be maintained within the food service establishment for review upon request by the Health Authority. Further, such cleaning and sanitizing plan for employees to follow in the event of encountering vomitus or fecal matter shall be included and submitted at the time of permit application as specified in DPH Rule 511-6-1-.02(1)(c). Adequate ventilation and lighting; designated areas used — Inspector Note: Observed a very strong odor in the women's restroom. PIC advised the smell. Please have the odor investigated by 8/04/2026. Email a copy of the completed work order and plan of action to resolve the odor by 08/04/2026. You can email the documentation to [REDACTED EMAIL]. Also observed a cell phone stored with condiments in the food preparation area. 511-6-1.07(4)(b) - Designated Areas for Employee Activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (C) (b) Designated Areas for Employee Activity. 1. Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. 2. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.
Food separated and protected — Inspector Note: Observed raw shell eggs stored next to cooked eggs and deli meet in the reach in cooler. Cracked eggshells and liquid was observed on the shelled egg cartons. Also observed raw shelled eggs stored next to containers of liquid pasteurized eggs in the walk in cooler. Requested a copy of the standard operating procedures for storing raw shelled eggs, Canadian bacon, and ready to heat eggs. Ms. Haynes could not access the standard operating procedures because it requires a paid subscription. Ms. Haynes will follow up with the District Manager (Darrell Glass)
Utensils, equipment and linens: properly stored, dried, handled — Inspector Note: Observed the coffee filter holder stored in the handsink. The coffee filter was stored prior to being washed in the three compartment sink. PIC advised to not store food contact surfaces in the handsink. PIC corrected on site.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed plastic used to protect equipment peeling beneath the lid in the reach in freezer used to store raw frozen burgers. PIC will have the plastic removed by 1/15/2026 to prevent physical contamination.

Insects, rodents, and animals not present — Inspector Note: Observed the vector control light stored above the bacon storage area. PIC will have the vector control light relocated by 1/20/2026 to prevent potential contamination. 511-6-1.07(2)(k) - Insect Control Devices (C) (k) Insect Control Devices, Design and Installation. 1. Insect control devices that are used to electrocute or stun flying insects shall be designed to retain the insect within the device. 2. Insect control devices shall be installed so that: (i) The devices are not located over a food preparation area; and (ii) Dead insects and insect fragments are prevented from contact with exposed food; clean equipment, utensils, and linens; and unwrapped single-service and single-use articles.
