Al vents are missing the center cover, leaving a hole opening in the ceiling. Observed a hole in the exhaust vent in the lobby area.

Insects, rodents, and animals not present — Inspector Note: Observed the back screen door ajar open. The screen door had a wooden frame and a mesh next stapled around it to create a screen door. The door was not attached to the hinges. PIC explained that the fire department told them to remove the original screen door due to it being a fire hazard because the screen door opened inward instead of outward. PIC was advised that all openings of the establishment and kitchen area must be sealed to prevent pest entry and activity.

The PIC simply wiped the blade of the knife if a rag, rinsed the knife, and did not sanitize. PIC was advised on how to properly wash, rinse, sanitize, and air dry the food equipment.
