The sink must be properly fixed and a handwashing sign must be added immediately. A employee must wash hands sign must be added to all handsinks including the men's restroom. 511-6-1.07(3)(d) - Handwashing Signage (C) (d) Handwashing Signage. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees. No handwashing sign provided at a handsink used by food employees.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed a splash guard needed between the handsink and three compartment sink drainboard in the bar area. PIC will have the splash guard installed by 6/1/26. 511-6-1.05(1)(i) - Nonfood-contact Surfaces (C) (i) Nonfood-contact Surfaces. Nonfood-contact surfaces of equipment that are exposed to splash, spillage, or other food soiling or that require frequent cleaning shall be constructed of a corrosion-resistant, nonabsorbent, and smooth material.
Warewashing facilities: installed, maintained, used; test strips — Inspector Note: Observed quaternary ammonium test strips provided by the PIC expired 4/15/2026. PIC discarded and replaced the test strips. 511-6-1.05(3)(h),(i),(j) - Temperature Measuring Device, Manual Warewashing; Sanitizing Solutions, Testing Device (Pf) (h) Temperature Measuring Devices, Manual and Mechanical Warewashing. 1. In manual warewashing operations, a temperature measuring device shall be provided and readily accessible for frequently measuring the washing and sanitizing temperatures. Pf 2. In hot water mechanical warewashing operations, an irreversible registering temperature indicator shall be provided and readily accessible for measuring the utensil surface temperature. Pf (i) Sanitizing Solutions, Testing Devices. A test kit or other device that accurately measures the concentration in mg/L of sanitizing solutions shall be provided. Pf j) Cleaning Agents and Sanitizers. 1. Cleaning agents that are used to clean equipment and utensils as specified under section (7) of this Rule, shall be provided and available for use during all hours of operation. Pf 2. Except for those that are generated on-site at the time of use, chemical sanitizers that are used to sanitize equipment and utensils, as specified under Section (8) of this Rule, shall be provided and available for use during all hours of operation. Nonfood-contact surfaces clean — Inspector Note: Observed build up on shelves in the cabinet. Also observed build up on the bottom of the preparation table above the area where mixing bowls are stored in the kitchen area. PIC will clean the area by 5/28/26.
Garbage/refuse properly disposed; facilities maintained — Inspector Note: Observed a lot of trash on the outside behind the dumpster on the dumpster pad. This area must be cleaned and maintained to reduce rodent and bug activity by 5/28/26. 511-6-1.06(5)(o),(p) - Using Drain Plugs; Maintaining Refuse Areas (C) (o) Using Drain Plugs. Drains in receptacles and waste handling units for refuse, recyclables, and returnables shall have drain plugs in place. (p) Maintaining Refuse Areas and Enclosures. A storage area and enclosure for refuse, recyclables, or returnables shall be maintained free of unnecessary items and clean. 511-6-1.06(5)(r),(s) - Frequency, removal of refuse, recyclables and returnables often enough to minimize odors and attracting pests; Receptacles or Vehicles, removal from premises in approved manner (C) (r) Frequency. Refuse, recyclables, and returnables shall be removed from the premises at a frequency that will minimize the development of objectionable odors and other conditions that attract or harbor insects and rodents. (s) Receptacles or Vehicles. Refuse, recyclables, and returnables shall be removed from the premises by way of: 1. Portable receptacles that are constructed and maintained according to law; or 2. A transport vehicle that is constructed, maintained, and operated according to law.
Physical facilities installed, maintained, and clean — Inspector Note: Observed syrup build up beneath the drink box storage area resulting from a leak. PIC will have the drink lines inspected to determine the source of the leak. 511-6-1.07(5)(a),(b) - Good Repair, physical facilities maintained; Cleaning, Frequency & Restrictions, cleaned often enough to keep them clean (C) (5) Maintenance and Operation. (a) Good Repair. All physical facilities shall be maintained in good repair. (b) Cleaning, Frequency and Restrictions. 1. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. 2. Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing. 3. If present, playground equipment and associated areas shall be maintained in a clean and sanitary condition. In addition, a cleaning and sanitizing plan along with cleaning and sanitizing records for playground equipment and associated areas shall be maintained within the food service establishment for review upon request by the Health Authority. Further, such cleaning and sanitizing plan for employees to follow in the event of encountering vomitus or fecal matter shall be included and submitted at the time of permit application as specified in DPH Rule 511-6-1-.02(1)(c). Adequate ventilation and lighting; designated areas used — Inspector Note: Observed build up on the kitchen vent and women's restroom vent.PIC will have both vents cleaned by 5/28/26 to prevent odors and potential contamination of food in the kitchen area.

Utensils, equipment and linens: properly stored, dried, handled — Inspector Note: Observed clean bowls in the kitchen and clear containers in the dish area not inverted or covered to protect from contamination. PIC started the process of inverted food contact surfaces (COS). Also the bulk ice bucket stored at the bar not inverted (COS). 511-6-1.05(10)(e)1,2,4 - Equipment, Utensil, Linens, stored 6" off floor in clean, dry location (C) (e) Equipment, Utensils, Linens, and Single-Service and Single-Use Articles. 1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor. 2. Clean equipment and utensils shall be stored as specified under paragraph 1 of this subsection and shall be stored: (i) In a self-draining position that allows air drying; and (ii) Covered or inverted. 4. Items that are kept in closed packages may be stored less than 6 inches (15 centimeters) above the floor on dollies, pallets, racks, and skids that are designed as specified under subsection (2)(hh) of this Rule.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed the legs missing on the handwashing sink at the bar. Also, the handwashing sink at the bar is not sealed to the wall. Equipment must be maintained in good repair. The legs must be replaced this is a consistent repair.
Physical facilities installed, maintained, and clean — Inspector Note: Observed rust spots on the ceiling of the walk in cooler. Observed stains on the ceiling in the dishwashing room. Please clean and seal the rust and stains so that contamination of food items will not occur.
Insects, rodents, and animals not present — Inspector Note: Observed gnats at the bar near the handwashing sink and in the men's restroom around the handwashing sink. Pest control will need to treat the facility by 12/9/25 for fruit flies. Make sure to keep floors dry and clean. Email a copy of the pest control report to [REDACTED EMAIL] by 12/10/25. 511-6-1.07(5)(k) - Controlling Pests (Pf, C) (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and 4. Eliminating harborage conditions.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed a thick black mold-like substance on the interior surface of the ice machine. Also, observed a thick accumulation of black mold build up on the interior surface of the ice scoop bowl. Please make sure to clean food contact surfaces regularly to sight and touch. 511-6-1.05(7)(a)1 - Equipment, Food-Contact Surfaces,& Utensils (Pf) (7) Cleaning of Equipment and Utensils. (a) Equipment, Food-Contact Surfaces, and Utensils. 1. Equipment food-contact surfaces and utensils shall be clean to sight and touch.
Proper date marking and disposition — Inspector Note: Observed expired sliced green peppers and hot dogs in the reach in cooler. The green peppers expired on 11/29 and the hot dogs on 11/26. Items were discarded (COS).
