2 recorded inspectionsContamination prevented during food preparation, storage, display — Inspector Note: Observed boxes of rice and vegetable oil stored under the preparation sink. 511-6-1.04(4)(q) - Food Storage (C) (q) Food Storage. 1. Except as specified in paragraphs 2 and 3 of this subsection, food shall be protected from contamination by storing the food: (i) In a clean, dry location; (ii) Where it is not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 cm) above the floor. 2. Food in packages and working containers may be stored less than 6 inches (15 cm) above the floor on case lot handling equipment if the equipment can be moved by hand or by conveniently available apparatuses such as hand trucks and forklifts. 3. Pressurized beverage containers, cased food in waterproof containers such as bottles or cans, and milk containers in plastic crates may be stored on a floor that is clean and not exposed to floor moisture.
Wiping cloths: properly used and stored — Inspector Note: Observed sanitizer buckets (Quat) did not meet minimum concentration of 200-400 ppm. Sanitizer was observed at 100 ppm. Once remade, the sanitizer was observed at 200 ppm
Plumbing installed; proper backflow devices — Inspector Note: Observed stagnant water on the floor in the dishwashing area due to the dishmachine leaking. PIC will place a work order. 511-6-1.06(2)(r) - System Maintained in Good Repair (P, C) (r) System Maintained in Good Repair. A plumbing system shall be repaired according to law; P and maintained in good repair.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed an employee drink with a lid stored next to toppings in the reach in cooler (Pizza Station). Also observed condensation build up on vents in the preparation and dishwashing area. 511-6-1.07(4)(b) - Designated Areas for Employee Activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (C) (b) Designated Areas for Employee Activity. 1. Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. 2. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.
Insects, rodents, and animals not present — Inspector Note: Observed the presence of mouse droppings in the CO2 drink storage area. 511-6-1.07(5)(l) - Removing Dead or Trapped Birds, Insects, Rodents, and Other Pest (C) (l) Removing Dead or Trapped Birds, Insects, Rodents, and Other Pests. Dead or trapped birds, insects, rodents, and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition, or the attraction of pests
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed dirty dishes stored in with clean dishes in the storage area. PIC corrected on site by rewashing the dishes.
Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: Observed employee must wash hands before returning to work signs not provided in the men's and ladies restroom. 511-6-1.07(3)(d) - Handwashing Signage (C) (d) Handwashing Signage. A sign or poster that notifies food employees to wash their hands shall be provided at all handwashing sinks used by food employees and shall be clearly visible to food employees.
Utensils, equipment and linens: properly stored, dried, handled — Inspector Note: Observed a grill press stored on an unclean surface below the fry hot holding station. PIC advised to store in a clean/dry location. 511-6-1.05(10)(e)1,2,4 - Equipment, Utensil, Linens, stored 6" off floor in clean, dry location (C) (e) Equipment, Utensils, Linens, and Single-Service and Single-Use Articles. 1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor. 2. Clean equipment and utensils shall be stored as specified under paragraph 1 of this subsection and shall be stored: (i) In a self-draining position that allows air drying; and (ii) Covered or inverted. 4. Items that are kept in closed packages may be stored less than 6 inches (15 centimeters) above the floor on dollies, pallets, racks, and skids that are designed as specified under subsection (2)(hh) of this Rule.
Single-use/single-service articles: properly stored, used — Inspector Note: Observed single use articles (lids) stored on the floor of the dry storage room. Violation Code Reference 511-6-1.05(10)(e)1&3 - Single-Service/Single-Use items stored 6" off floor in clean, dry location (C) (e) Equipment, Utensils, Linens, and Single-Service and Single-Use Articles. 1. Except as specified in paragraph 4 of this subsection, cleaned equipment and utensils, laundered linens, and single-service and single-use articles shall be stored: (i) In a clean, dry location; (ii) Where they are not exposed to splash, dust, or other contamination; and (iii) At least 6 inches (15 centimeters) above the floor. 3. Single-service and single-use articles shall be stored as specified under paragraph 1 of this subsection and shall be kept in the original protective package or stored by using other means that afford protection from contamination until used.
Nonfood-contact surfaces clean — Inspector Note: Observed build up (saw dust) and black accumulation in the cabinets below the drink stations. There were cookies also stored in the cabinet. 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. 3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
Plumbing installed; proper backflow devices — Inspector Note: Observed the mop sink faucet leaking causing water accumulation on the floor around the mop sink. PIC repair the plumbing leak by 2/10/26. 511-6-1.06(2)(r) - System Maintained in Good Repair (P, C) (r) System Maintained in Good Repair. A plumbing system shall be repaired according to law; P and maintained in good repair.
Physical facilities installed, maintained, and clean — Inspector Note: Observed the floor in the dry storage room in bad repair. There were several area that contained floor tile in disrepair. PIC will have the floors repaired by 2/10/26 to prevent injury and water accumulation. There was also rust build up in the CO2 tank storage area.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed build up on vents in the dry storage and clean dish storage area. PIC will have the vents clean by 2/03/26 to prevent potential contamination. 511-6-1.07(2)(j) - Heating, Ventilation, Air Conditioning, System Vents, designed to prevent contamination (C) (j) Heating, Ventilating, Air Conditioning, System Vents. Heating, ventilating, and air conditioning systems shall be designed and installed so that make-up air intake and exhaust vents do not cause contamination of food, food-contact surfaces, equipment, or utensils.
Proper hot holding temperatures — Inspector Note: Observed containers of vegetable soup stored in the hot holding steam well did not maintain a temperature of 135 degrees or higher. The vegetable soup was observed at 126 degrees f. PIC corrected onsite by reheating to 165 degrees f. 511-6-1.04(6)(f) - Time/Temperature Control for Safety; Hot Holding (P) (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above.
