Physical facilities installed, maintained, and clean — Inspector Note: Observed floor tiles under the front food service area missing and damaged. This violation was also noted in their previous pest control inspection report from 4/22/2026. Observed a hole in the ceiling tile in the back storage room where beverage containers and linen are stored.

PIC was advised that the food safety manager may only had a certification for one location.

Item 11A Good Retail Practice

Proper cooling methods used: adequate equipment for temperature control — Inspector Note: Observed improper cooling methods being used to cool TCS foods. Observed a hotel pan of chicken that was cooked at 11:00 am today on the bottom shelf covered during cooling in the walk in cooler. CA- PIC reduced the portion size and relocated to the second shelf from the top. PIC will ensure a lid or cover is placed above the shelf where product is cooling. The cooked chicken did meet temperature requirements during cooling.

Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: Observed no employee must wash hands sign was provided in the men's restroom. PIC will replace the sign by 11/13/25.
