Contamination prevented during food preparation, storage, display — Inspector Note: Observed fries stored uncovered in the reach in cooler in the lowboy cooler. The fries were stored beneath an area that contained build up. Also observed a sheet pan of bread stored next to a chemical bucket in the front of house. PIC corrected on site. 511-6-1.04(4)(t) - Food Preparation (C) (t) Food Preparation. During preparation, unpackaged food shall be protected from environmental sources of contamination.

Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed a splash guard must be installed in the front of house on the handsink to prevent potential contamination from splashing. Heat strip not working on the walk in freezer door causing ice build up. Also observed the reach in cooler door on the preparation line in bad repair. PIC will have the cooler repaired by 5/23/2025. 511-6-1.05(1)(i) - Nonfood-contact Surfaces (C) (i) Nonfood-contact Surfaces. Personal cleanliness — Inspector Note: Observed several employees in the kitchen without hair or beard restraints to properly restrain hair. Violation Code Reference 511-6-1.03(5)(j) - Hair Restraints (C) (i) Hair Restraints. 1. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. 2. This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles.

Toxic substances properly identified, stored, used — Inspector Note: Observed an opened bottle of restroom cleaner stored above the drink syrups and right beside the tea concentrate packages. Observed restroom cleaner and another chemical stored by single use forks underneath the cabinet on the front-end. Advised PIC to make sure all chemicals are stored in a designated area after use. All chemicals were removed and placed in the designated area (COS). 511-6-1.07(6)(c) - Storage, Separation (P) (c) Storage, Separation.
