Proper cooling methods used: adequate equipment for temperature control — Inspector Note: Observed Philly chicken cooling improperly. The philly chicken waws observed in a container with a lid. 511-6-1.04(6)(e) - Cooling Methods (Pf, C) (e) Cooling Methods. 1. Cooling shall be accomplished in accordance with the time and temperature criteria specified under DPH Rule 511-6-1-.04(6)(d) by using one or more of the following methods depending on the type of food being cooled: (i) Placing the food in shallow pans; Pf (ii) Separating the food into smaller or thinner portions; Pf (iii) Using rapid cooling equipment; Pf (iv) Stirring the food in a container placed in an ice water bath; Pf (v) Using containers that facilitate heat transfer; Pf (vi) Adding ice as an ingredient; Pf or (vii) Other effective methods. Pf 2. When placed in cooling or cold holding equipment, food containers in which food is being cooled shall be: (i) Arranged in the equipment to provide maximum heat transfer through the container walls; and (ii) Loosely covered, or uncovered if protected from overhead contamination during the cooling period to facilitate heat transfer from the surface of the food.

Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed unsealed wooden shelves where seasonings are stored. Seal shelves with polyurethane coating.

Plumbing installed; proper backflow devices — Inspector Note: Observed the hot water faucet leaking on the 3 compartment sink. Have fixtures repaired. Also observed the HVAC system is leaking into a bucket. PIC will have the system inspected and repaired by 8/1/2026. Observed the cold water handle on the handwashing sink stripped and not turning on properly. 511-6-1.06(2)(r) - System Maintained in Good Repair (P, C) (r) System Maintained in Good Repair. A plumbing system shall be repaired according to law; P and maintained in good repair.

Garbage/refuse properly disposed; facilities maintained — Inspector Note: Observed several mattresses store in and around the dumpster. Mattresses must be removed by 7/25/26 to prevent pest activity.

Physical facilities installed, maintained, and clean — Inspector Note: Observed ceiling tiles missing at back door and wall with holes in them behind reach-in cooler, and wood in need of sealing. Ceiling tile in the room located in the vestibule area contained stained tiles from a prior leak. PIC will contact the landlord to have the leak repaired prior to changing ceiling tile. 511-6-1.07(2)(a) - Floor, Walls, & Ceilings, Cleanability (C) (2) Design, Construction, and Installation. (a) Floors, Walls, and Ceilings, Cleanability. Except as specified under subsection (2)(d) of this Rule and except for antislip floor coverings or applications that may be used for safety reasons, floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.

Adequate ventilation and lighting; designated areas used — Inspector Note: Observed the hood system not working properly causing smoke build up in the kitchen and dining area. PIC advised the hood system is being repaired. A completed work order will be emailed to [REDACTED EMAIL] and [REDACTED EMAIL] by 7/26/2026. Also observed lights flickering in the kitchen and lobby area.
