(a) Equipment and Utensils, Air-Drying Required. After cleaning and sanitizing, equipment and utensils: 1. Shall be air-dried or used after adequate draining before contact with food; and 2. May not be cloth dried except that utensils that have been air-dried may be polished with cloths that are maintained clean and dry.

Nonfood-contact surfaces clean — Inspector Note: Observed build up (mold like substance) in caulking around the three compartment sink and bar sink. Physical facilities installed, maintained, and clean — Inspector Note: Observed build up on ceilings and ceiling vents in the kitchen and bar areas. 511-6-1.07(5)(a),(b) - Good Repair, physical facilities maintained; Cleaning, Frequency & Restrictions, cleaned often enough to keep them clean (C) (5) Maintenance and Operation. (a) Good Repair. All physical facilities shall be maintained in good repair. (b) Cleaning, Frequency and Restrictions. 1. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. 2. Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing. 3. If present, playground equipment and associated areas shall be maintained in a clean and sanitary condition. In addition, a cleaning and sanitizing plan along with cleaning and sanitizing records for playground equipment and associated areas shall be maintained within the food service establishment for review upon request by the Health Authority. Proper cold holding temperatures — Inspector Note: Observed TCS foods (crabsticks, eel, EBI shrimp, smoked salmon, white rice, raw beef tenderloin, raw chicken, and raw shrimp) did not meet temperature requirements of 41 degrees or below for cold holding. Low boy cooler was also observed at 46 degrees F. PIC discarded all TCS foods that were out of temperature. 511-6-1.04(6)(f) - Time/Temperature Control for Safety; Cold Holding (P) (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except as specified under paragraph 4 of this subsection, if time without temperature control is used as the public health control for a working supply of time/temperature control for safety food before cooking, or for ready-to-eat time/temperature control for safety food that is displayed or held for sale or service, written procedures shall be prepared in advance, maintained in the food service establishment, and made available to the regulatory authority upon request that specify: Pf (i) Methods of compliance with paragraphs 2(i) - (iv) or 3(i) through (v) of this subsection; and (ii) Methods of compliance with the cooling of time/temperature control for safety food that is prepared, cooked, and refrigerated before time is used as a public health control. The tables must be treated for rust, cleaned, and resealed or replaced.

Physical facilities installed, maintained, and clean — Inspector Note: Observed the ceiling tile stained in several areas throughout the kitchen. Observed the ceiling tile with a dust accumulation by the 3-compartment sink. Observed debris underneath the dry storage shelves. Make sure to clean the floors regularly.

Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. 2. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.

PIC stated that the presence of drain flies have gotten better over time. Pest control records were reviewed.
