A thermometer must be placed in the warmest part of each cooler. 511-6-1.05(2)(x) - TMD, Located in Refrigerators/ Hot Holding (Pf, C) (x) Temperature Measuring Devices. 1. In a mechanically refrigerated or hot food storage unit, the sensor of a temperature measuring device shall be located to measure the air temperature in the warmest part of a mechanically refrigerated unit and in the coolest part of a hot food storage unit. 2. Except as specified in paragraph 3 of this subsection, cold or hot holding equipment used for time/temperature control for safety food shall be designed to include and shall be equipped with at least one integral or permanently affixed temperature measuring device that is located to allow easy viewing of the device's temperature display. 3. The requirement in paragraph 2 of this subsection does not apply to equipment for which the placement of a temperature measuring device is not a practical means for measuring the ambient air surrounding the food because of the design, type, and use of the equipment such as insulated food transport containers and salad bars. 4. Temperature measuring devices shall be designed to be easily readable. 5. Food temperature measuring devices and water temperature measuring devices on warewashing machines shall have a numerical scale, printed record, or digital readout in increments no greater than 1ºC or 2ºF in the intended range of use. Wiping cloths: properly used and stored — Inspector Note: Observed two sanitizer buckets (Quat) did not meet the minimum required concentration of 200-400 ppm while stored in the food preparation area. PIC corrected on site by remaking the sanitizer buckets to 200 ppm. 511-6-1.04(4)(m) - Wiping Cloths, Use Limitation (C) (m) Wiping Cloths, Use Limitation. 1. Cloths in-use for wiping food spills from tableware and carry-out containers that occur as food is being served shall be: (i) Maintained dry; and (ii) Used for no other purpose. 2. Cloths in-use for wiping counters and other equipment surfaces shall be: (i) Held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n); and (ii) Laundered daily. 3. Cloths in-use for wiping surfaces in contact with raw animal foods shall be kept separate from cloths used for other purposes. 4. Dry wiping cloths and the chemical sanitizing solutions in which wet wiping cloths are held between uses shall be free of food debris and visible soil. 5. Containers of chemical sanitizing solutions in which wet wiping cloths are held between uses shall be stored off the floor and used in a manner that prevents contamination of food, equipment, utensils, linens, single-service, or single-use articles. 6. Single-use disposable sanitizer wipes shall be used in accordance with EPA-approved manufacturer’s label use instructions.
Nonfood-contact surfaces clean — Inspector Note: Rust observed on the exterior of the ice machine. Also observed grease accumulation and old food build up below the deep fryers
Plumbing installed; proper backflow devices — Inspector Note: Observed a leak below the three compartment sink. The handsink located next to the three compartment sink was not working. 511-6-1.06(2)(r) - System Maintained in Good Repair (P, C) (r) System Maintained in Good Repair. A plumbing system shall be repaired according to law; P and maintained in good repair.
Physical facilities installed, maintained, and clean — Inspector Note: Observed floor grout missing and in bad repair throughout the kitchen. Also observed several holes in the wall and the baseboard in bad repair in the dish washing area and restrooms. The thresh hold between the storage room and walk in cooler is missing. PIC will have the areas repaired by 7/13/26. Also observe water build up on floors. 511-6-1.07(5)(a),(b) - Good Repair, physical facilities maintained; Cleaning, Frequency & Restrictions, cleaned often enough to keep them clean (C) (5) Maintenance and Operation. (a) Good Repair. All physical facilities shall be maintained in good repair. (b) Cleaning, Frequency and Restrictions. 1. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. 2. Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing. 3. If present, playground equipment and associated areas shall be maintained in a clean and sanitary condition. In addition, a cleaning and sanitizing plan along with cleaning and sanitizing records for playground equipment and associated areas shall be maintained within the food service establishment for review upon request by the Health Authority. Further, such cleaning and sanitizing plan for employees to follow in the event of encountering vomitus or fecal matter shall be included and submitted at the time of permit application as specified in DPH Rule 511-6-1-.02(1)(c). Adequate ventilation and lighting; designated areas used — Inspector Note: Observed severe build up on vents throughout the kitchen and dining area. This is a repeat and ongoing issue. A mold remediation company must follow up by 7/12/26 and test/treat the location for mold. PIC will have all vents and ducts inspected and professionally treated by 7/12/2026. A completed service work order and plan of action must be sent to [REDACTED EMAIL] by 07/12/2026. Also observed severe mold on vents in the restroom and above the drink storage area in the dishwashing area. If the mold issue is not tested, treated, and remediated, the location will be required to close on 7/13/26 due to the potential imminent health hazard. A warning notice was issued. 511-6-1.07(5)(d) - Cleaning Ventilation System, Nuisance & Discharge Prohibition, cleaned in way not to cause contamination or create a public health hazard (C) (d) Cleaning Ventilation Systems, Nuisance and Discharge Prohibition. Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination by dust, dirt, and other materials. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.
Proper cold holding temperatures — Inspector Note: Observed several TCS foods including hot dogs, 1 percent low fat milk, raw shelled eggs and liquid pasteurized eggs did not meet temperature requirements of 41 degrees or below in the walk in cooler. PIC discarded all product that did not meet temperature requirements. PIC will have the walk in cooler checked by maintenance due to the electrical breaker being off initially. PIC advised that the location has had an issue with the electrical box since the last storm. PIC will email a copy of the service work order to [REDACTED EMAIL] and [REDACTED EMAIL] by 7/10/2026. 511-6-1.04(6)(f) - Time/Temperature Control for Safety; Cold Holding (P) (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. Personal cleanliness — Inspector Note: Observed staff member with long painted fingernails in the food preparation area not wearing gloves. CA- Staff member washed hands used gloves as a corrective action. 511-6-1.03(5)(f) - Fingernails (Pf) (f) Fingernails. Employees shall keep their fingernails clean and trimmed to no longer than the tips of the fingers.
