Physical facilities installed, maintained, and clean — Inspector Note: Observed baseboards missing in the storage area of the kitchen. All repairs must be completed to prevent bugs from entering the kitchen. The wall in the storage room must be sealed or painted with a semi gloss paint.

Food separated and protected — Inspector Note: Observed raw shelled eggs stored above pre cooked and raw bacon in the reach in cooler. PIC will ensure that all food items are stored correctly according to cooking temperatures. Food in good condition, safe, and unadulterated — Inspector Note: Observed the presence of green, red, and yellow peppers stored in a box in the reach in cooler. Red peppers were in bad condition. PIC corrected on site by discarding the box of peppers. Insects, rodents, and animals not present — Inspector Note: Observed live roaches in the cabinent below the drinkstation. Spoke wih the PIC and Major. A work order will be placed with pest control to trat the area for the next 30 days. A follow up will be conducted on 10/4/25. Please email a copy of the pest control service report each week for 30 days. 511-6-1.07(5)(k) - Controlling Pests (Pf, C) (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and 4. Eliminating harborage conditions.
