PIC will obtain a beard guard by 6/10/26. 511-6-1.03(5)(j) - Hair Restraints (C) (i) Hair Restraints. 1. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. 2. This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles.
Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: Observed the current food permit and choking poster not posted. The food permit posted expired on 1/1/2026. PIC will obtain and post the choking poster by 6/10/26.
Plumbing installed; proper backflow devices — Inspector Note: Observed a condensation leak coming from the bottom of the reach in cooler. PIC using a bread pan to catch water. Stagnant water observed on the floor. PIC will place a work order and email to robert.lewis`@dph.ga.gov by 6/10/26 Observed the hot water shut off on the vegetable prep sink. PIC stated that the store no longer uses the sink. Once the sink was turned on the hot water would not shut off manually. As long as the sink is in the facility it has to be maintained and have both hot and cold water. The sink must be serviced by a plumber and repaired by 06/10/26. This is a repeat violation. 511-6-1.06(2)(r) - System Maintained in Good Repair (P, C) (r) System Maintained in Good Repair. A plumbing system shall be repaired according to law; P and maintained in good repair.
Toilet facilities: properly constructed, supplied, cleaned — Inspector Note: Observed the unisex restroom door without a self closing device. Self closure must be installed by 6/15/26. 511-6-1.07(2)(l) - Toilet Rooms, Enclosed (C) (l) Toilet Rooms, Enclosed. Except where a toilet room is located outside a food service establishment and does not open directly into the food service establishment, such as a toilet room that is provided by the management of a shopping mall, a toilet room located on the premises shall be completely enclosed and provided with a tight-fitting and self-closing door. 511-6-1.07(2)(l) - Toilet Rooms, Enclosed (C) (l) Toilet Rooms, Enclosed. In-use utensils: properly stored — Inspector Note: Observed a long fork stored on top of the pizza oven. The pizza oven had an accumulation of dust build up on it. During pauses utensils should be stored in a clean area. The fork was removed (COS). 511-6-1.04(4)(k) - In-Use Utensils, Between-Use Storage (C) (k)Storage of In-Use Utensils. During pauses in food preparation or dispensing, food preparation and dispensing utensils shall be stored: 1.Except as specified under paragraph 2 of this subsection, in the food with their handles above the top of the food and the container; 2.In food that is not time/temperature control for safety food with their handles above the top of the food within containers or equipment that can be closed, such as bins of sugar, flour, or cinnamon; 3.On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized at a frequency specified under DPH Rule 511-6-1-.05(7)(b) and (8)(a); 4.In running water of sufficient velocity to flush particulates to the drain, if used with moist food such as ice cream or mashed potatoes; 5. In a clean, protected location if the utensils, such as ice scoops, are used only with a food that is not time/temperature control for safety food; or 6. In a container of water if the water is maintained at a temperature of at least 135°F (57°C) and the container is cleaned at a frequency specified under DPH Rule 511-6-1-.05(7)(b)3(vi).
Nonfood-contact surfaces clean — Inspector Note: Observed an accumulation of build up on the exterior surface of the pizza oven next to the oven's rollers. 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. 3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Proper date marking and disposition — Inspector Note: Observed a container of shredded two cheese with an expiration date of 12/7. It was prepped on 11/30. Advised PIC to discard the cheese and make sure to rotate and monitor food products (COS).
