1 recorded inspectionWiping cloths: properly used and stored — Inspector Note: Observed the sanitizer buckets with towels in it at 0 PPM quaternary ammonium solution. PIC remade the solution to 200 PPM quaternary ammonium (COS).
Toilet facilities: properly constructed, supplied, cleaned — Inspector Note: Observed the toilet in the men's restroom in need of cleaning due to stains on the interior surface. 511-6-1.07(5)(h) - Cleaning Plumbing Fixtures- Handsinks, Urinals & Toilets (C) (h) Cleaning of Plumbing Fixtures. Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary to keep them clean and maintained.
Physical facilities installed, maintained, and clean — Inspector Note: Observed the floors in the dry storage room, walk in freezer, and underneath the fryers in need of a deep cleaning due to accumulation. The floor in the dry storage area is in bad repair and must be properly sealed. Observed the wall in need of a resealing or a deep cleaning behind the replacement drink syrups due to an accumulation of drink spills. Also, the wall behind the 3-compartment sink has an accumulation of black mold that must be cleaned. Observed the caulking around the 3-compartment sink in need of removing and resealing due to a thick black mold-like accumulation. Observed the floor tile in bad repair by the back door and underneath the fryers. The tile must be properly grouted and sealed to make the areas smooth and easily cleanable. Observed the baseboards missing around the walls in the walk in cooler. The baseboards must be added around the walls. Observed the ceiling tile in the hot area in need of cleaning or replacing due to heavy staining (dark brown, yellow, and dust build up). All violations are a consecutive repeat, failure to correct will result in the removal of the permit.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed a jacket and a hat handing on a rack above loaves of bread. All employees items must be stored in a designated area. PIC removed the items (COS). 511-6-1.07(4)(b) - Designated Areas for Employee Activity, located to prevent contamination of food, equipment, utensils, linens, & single service articles (C) (b) Designated Areas for Employee Activity. 1. Areas designated for employees to eat, drink, use tobacco products and electronic devices shall be located so that food, equipment, linens, and single-service and single-use articles are protected from contamination. 2. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed several HVAC vents throughout the kitchen with black stains and rust build up. All vents must be cleaned and repainted to avoid contamination of food items. This is a consecutive repeat.
Food separated and protected — Inspector Note: Observed raw chicken tenders stored directly beside precooked omelet mix & raw shelled eggs stored above hot dogs and sausage in the reach in cooler beside the grill. PIC removed the items and arranged them according to cooking temperatures (COS). 511-6-1.04(4)(c)1(i)(ii)(iii)(v)(vi)(vii)(viii) - Packaged & Unpackaged Food Separation, Packaging, and Segregation (P, C) (c) Packaged and Unpackaged Food - Separation, Packaging, and Segregation. 1. Food shall be protected from cross contamination by: (i) Except as specified in paragraph (i)(IV) of this subsection, separating raw animal foods during storage, preparation, holding, and display from: (I) Raw ready-to-eat food including other raw animal food such as fish for sushi or molluscan shellfish, or other raw ready-to-eat food such as fruits and vegetables, P and (II) Cooked ready-to-eat food; and P (III) Fruits and vegetables before they are washed; P (IV) Frozen, commercially processed and packaged raw animal food may be stored or displayed with or above frozen, commercially processed and packaged, ready-to-eat food. Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed the ice machine with a black and brown build up on the interior roof. Advised PIC to clean the ice machine, so that it is clean to the sight and touch.
