Observed staff members not wearing proper hair restraints while preparing food in the food preparation area. CA- PIC will obtain effective hair restraints for team members by 9/17/25. 511-6-1.03(5)(i) - Hair Restraints (C) (i) Hair Restraints. 1. Employees preparing or handling food shall use effective and clean, disposable or easily cleanable nets or other hair restraints approved by the Health Authority, worn properly to restrain loose hair including beards and mustaches longer than one half inch. 2. This does not apply to employees such as counter staff who only serve beverages and wrapped or packaged foods, hostesses, and wait staff if they present a minimal risk of contaminating exposed food, clean utensils and linens and unwrapped single-service and single-use articles.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed mold build up in the caulking above the three-compartment sink. Also observed rust build up on shelves in the walk in cooler. CA- Shelves will be replaced by 11/16/25. Observed the door corner guard in bad repair in the walk in cooler.
Nonfood-contact surfaces clean — Inspector Note: Observed build up and trash below the drink station. Also observed build up on the interior of the reach in cooler. The cooler is out of order.
Toilet facilities: properly constructed, supplied, cleaned — Inspector Note: Observed the men's restroom toilet is out of order. PIC will repair the toilet by 9/18/25. PIC advised to email a copy of the completed work order for toilet repair to [REDACTED EMAIL] by 9/17/25 511-6-1.06(2)(h) - Toilets and Urinals, Numbers and Capacities (C) (h) Toilets and Urinals, Numbers and Capacities. 1. Toilet facilities shall be provided for food employees. 2. All toilet facilities shall be installed in accordance with applicable State or local plumbing code as amended, and shall be the number required by such code. 3. In toilet facilities that have exit doors with handles or knobs that must be touched to open, sanitary towels must be provided. 4. In all establishments with dining facilities on the premises and permitted since July 31, 1995, patrons' toilet facilities shall be provided. Access to patrons' toilet facilities shall not be through food service, preparation, storage, or warewashing areas. 5. When not on the same premises, the location of t Toilets shall be located on or within 200 feet of the premises. Off-premises toilets must be and approved by the Health Authority.
Physical facilities installed, maintained, and clean — Inspector Note: Observed stained ceiling tile in the dining area from a previous roof leak. PIC advised the leak was caused by the AC unit. The source of the leak has been repaired. PIC will change the ceiling tile. Observed baseboard missing in the walk in cooler.
