Posted: Permit/Inspection/Choking Poster/Handwashing — Inspector Note: Observed expired food service permit. Please contact the health department at [REDACTED PHONE] ext. 0 to pay for the permit and late fee. Owner paid ML Business Consulting to handle their permitting, but health department did not receive funds. Manager was advised to contact the health department directly to pay for permit fees for restaurant, base of operation, and mobile unit.

In-use utensils: properly stored — Inspector Note: Observed ice scoop handle stored in the ice in ice machine. PIC was advised that the handle should always be sticking out of the ice to prevent contamination of ice.

Adequate ventilation and lighting; designated areas used — Inspector Note: Observed employee personal sprite can (opened) and water bottles stored with dry good and single use articles. Observed employee sleeping cot inside of the single-use articles and dry goods storage room. PIC was advised that employees are not allowed to lounge in areas where food and single use articles are stored. PIC removed the cot from the storage room.

Toxic substances properly identified, stored, used — Inspector Note: Observed open container of bleach and Bonid Multi-Purpose cleaner stored with onions and potatoes on to of the reach in freezer. Manager removed the chemicals to it's designated area for chemicals.
