Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed debris on the floors in the walk-in freezer underneath the storage shelves. Door hinges must be adjusted or a new self-closing device must be put in place to allow walk in cooler door to self-close.
Nonfood-contact surfaces clean — Inspector Note: Observed mold like substance and dirt build up around the caulking of the 3 compartment sink. Please have old caulk removed, clean the area, and re-caulk the 3 compartment sink. Observed pizza rack with rust and food build up. Observed food accumulation on the catch tray at the bottom of the pizza oven.
Physical facilities installed, maintained, and clean — Inspector Note: Floors and walls in the ware washing area must be deep cleaned with special attention underneath the 3-compartment sinks. Observed broken floor tiles in the ware washing area along with damaged and missing baseboards.
Insects, rodents, and animals not present — Inspector Note: Observed large and small flies in the ware washing area.
