Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed self-closing device for the walk in cooler door in bad repair causing the door to not fully close. PIC was advised to manually close the door to ensure that it is completely closed and to email photos of the repaired self-closing device to [REDACTED EMAIL].

Hot and cold water available; adequate pressure — Inspector Note: Observed hand sink in the middle of the kitchen with hot water temperature of 76F. Manager was advised that the minimum hot water temperature must be at least 85F. Manager was also advised to utilize hand sanitizer after washing hands until hot water is restored in the hand sink. Hot water must be restored no later than July 27, 2026.

Plumbing installed; proper backflow devices — Inspector Note: Observed front hand sink leaking due to a missing equipment on the pipe underneath. Manager placed a do not use sign on the hand sink until repairs are made. Repairs must be completed by July 27, 2026. Observed middle hand sink in need of caulking around the splash guards to prevent water from leaking onto the floor from in between the sink and splash guards.

(a) Good Repair and Proper Adjustment. 1. Equipment shall be maintained in a state of repair and condition that meets the requirements specified under subsections (1) and (2)of this Rule. 2. Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. 3. Cutting or piercing parts of can openers shall be kept sharp to minimize the creation of metal fragments that can contaminate food when the container is opened.

Observed food debris on the exterior surface of the diced fruit container at the front service counter. Observed the lemonade dispenser with excessive build up at the front service counter. Observed the counter underneath the soda machine in need of cleaning due to water and beverage residue. 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. 3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.

Dressing rooms shall be used by employees if the employees regularly change their clothes in the establishment. 2. The manager was contacted via phone but the PIC and explained that he would contact the manager of Walmart to have a pest control service to come out for the fruit flies. Manager was advised to email pest control report for the additional treatment for the fruit flies to [REDACTED EMAIL]. 511-6-1.07(5)(k) - Controlling Pests (Pf, C) (k) Controlling Pests. The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by: 1. Routinely inspecting incoming shipments of food and supplies; 2. Routinely inspecting the premises for evidence of pests; 3. Using methods, if pests are found, such as trapping devices or other means of pest control as specified under subsections (6)(e), (6)(m), and (6)(n) of this Rule; Pf and 4. Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed interior ceiling of ice machine with black and pink mold-like substance. PIC (person in charge) was advised to turn off the ice machine, burn all the ice, and to clean and sanitize the interior surfaces of the ice machine.
