Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food.
Wiping cloths: properly used and stored — Inspector Note: Observed wiping cloth stored in a sanitizer solution below the proper minimum concentration. PIC remade sanitizer solution to proper minimum concentration. 511-6-1.04(4)(m) - Wiping Cloths, Use Limitation (C) (m) Wiping Cloths, Use Limitation. 1. Cloths in-use for wiping food spills from tableware and carry-out containers that occur as food is being served shall be: (i) Maintained dry; and (ii) Used for no other purpose. 2. Cloths in-use for wiping counters and other equipment surfaces shall be: (i) Held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6)(n); and (ii) Laundered daily. 3. Cloths in-use for wiping surfaces in contact with raw animal foods shall be kept separate from cloths used for other purposes. 4. Dry wiping cloths and the chemical sanitizing solutions in which wet wiping cloths are held between uses shall be free of food debris and visible soil. 5. Containers of chemical sanitizing solutions in which wet wiping cloths are held between uses shall be stored off the floor and used in a manner that prevents contamination of food, equipment, utensils, linens, single-service, or single-use articles. 6. Single-use disposable sanitizer wipes shall be used in accordance with EPA-approved manufacturer’s label use instructions.
Food and nonfood-contact surfaces cleanable, properly designed, constructed, and used — Inspector Note: Observed interior roof of microwave in disrepair. 511-6-1.05(6)(c) - Microwave Ovens (C) (c) Microwave Ovens. Microwave ovens shall be in good repair and meet the safety standards specified in 21 CFR 1030.10 for Microwave ovens.
Warewashing facilities: installed, maintained, used; test strips — Inspector Note: Observed facility utilizing quaternary ammonium sanitizer solution without proper test strips. PIC was advised to maintain test strips for quaternary ammonium sanitizer solution. 511-6-1.05(6)(p) - Warewashing Equipment, Determining Chemical Sanitizer Concentration (Pf) (p) Warewashing Equipment, Determining Chemical Sanitizer Concentration. Concentration of the sanitizing solution shall be accurately determined by using a test kit or other device. Pf.
Nonfood-contact surfaces clean — Inspector Note: Observed interior of reach-in cooler at the bar with excessive water accumulation and black accumulation on reach-in cooler shelving unit in main kitchen. 511-6-1.05(7)(a)2,3 - Equipment, Food/Nonfood-Contact Surfaces, and Utensils, food-contact surfaces of cooking equipment & nonfood-contact surfaces free of accumulations (C) 2. The food-contact surfaces of cooking equipment and pans shall be kept free of encrusted grease deposits and other soil accumulations. 3. Nonfood-contact surfaces of equipment shall be kept free of an accumulation of dust, dirt, food residue, and other debris.
Plumbing installed; proper backflow devices — Inspector Note: Observed evidence of walk-in freezer refrigeration unit piping leaking ( icicle formation on piping). 511-6-1.06(2)(r) - System Maintained in Good Repair (P, C) (r) System Maintained in Good Repair. Garbage/refuse properly disposed; facilities maintained — Inspector Note: Observed wall of outside dumpster enclosure area in disrepair. 511-6-1.06(5)(k),(l) - Storing Refuse, Recyclables & Receptacles; Areas, Enclosures, & Receptacles, Good Repair (C) (l) Areas, Enclosures, and Receptacles, Good Repair. Storage areas, enclosures, and receptacles for refuse, recyclables, and returnables shall be maintained in good repair.
Physical facilities installed, maintained, and clean — Inspector Note: Observed floor tiles in dish room and throughout the facility cracked and chipped and in need or recaulking. The floors in front of operational front of service drink machine were observed in need of resealing as well as the stairs adjacent to alcohol fridge. Also observed ceiling with rusting above dish machine and stagnant water throughout the facility. 511-6-1.07(5)(a),(b) - Good Repair, physical facilities maintained; Cleaning, Frequency & Restrictions, cleaned often enough to keep them clean (C) (5) Maintenance and Operation. (a) Good Repair. All physical facilities shall be maintained in good repair. (b) Cleaning, Frequency and Restrictions. 1. The physical facilities shall be cleaned as often as necessary to keep them clean and by methods that prevent contamination of food products. 2. Except for cleaning that is necessary due to a spill or other accident, cleaning shall be done during periods when the least amount of food is exposed, such as after closing.
Adequate ventilation and lighting; designated areas used — Inspector Note: Observed black mold-like accumulation on displaced, vent above clean dishes adjacent to dish machine. 511-6-1.07(2)(j) - Heating, Ventilation, Air Conditioning, System Vents, designed to prevent contamination (C) (j) Heating, Ventilating, Air Conditioning, System Vents. Heating, ventilating, and air conditioning systems shall be designed and installed so that make-up air intake and exhaust vents do not cause contamination of food, food-contact surfaces, equipment, or utensils.
Food-contact surfaces: cleaned & sanitized — Inspector Note: Observed can opener blade with old food accumulation and chipping metal coating. Proper cold holding temperatures — Inspector Note: Observed multiple TCS food items in prep top and reach-in cooler above 41F. PIC discarded of items in reach-in cooler and placed items in prep top cooler on ice. 511-6-1.04(6)(f) - Time/Temperature Control for Safety; Hot Holding (P) (f) Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above.
